About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as a Full Time Hospice Registered Nurse. As a Hospice Case Manager, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Manage a caseload of hospice patients in the home setting
  • Provide skilled nursing treatments, including wound care, medication management, and patient education
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Communicate effectively with patients, families, and healthcare providers
Requirements
  • Current state RN license
  • At least 1 year of experience working with adult and geriatric medical/surgical patients in a hospital or SNF setting
