LPN Crisis Care Role

We are seeking a compassionate and skilled LPN to join our team as a Crisis Care LPN. In this role, you will provide high-quality nursing care to patients in their homes or facilities, working under the direction of an RN.

Key Responsibilities:
  • Provide 1:1 care to patients at the bedside, focusing on their physical and emotional needs.
  • Work collaboratively with the patient's family and healthcare team to ensure comprehensive care.
  • Document patient care and services provided, using computer skills to maintain accurate records.
  • Participate in ongoing education and training to enhance your skills and knowledge in hospice care.
Requirements:
  • LPN licensure in the State of Ohio, with no board actions.
  • One year of nursing experience, preferably in hospice care.
  • CPR certification.
  • Computer skills sufficient to document patient care and services.
  • Valid driver's license with a safe driving record.
  • State minimum automobile insurance coverage.
