We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hospice of New York. As an LPN, you will play a vital role in providing end-of-life care to our patients and their families.

Responsibilities:

  • Provide routine patient visits and continuous care services
  • Deliver in-home respite care and personal care for patients
  • Administer wound care and provide On-Call coverage when needed
  • Collaborate with our RN team to ensure high-quality patient care

Requirements:

  • LPN licensed in NY State
  • Full Covid-19 Vaccination
  • 2+ years of LPN experience
  • Reliable motor vehicle required for patient visits
  • Ability to work with limited supervision
  • Strong interpersonal skills and a collaborative personality
  • Bilingual Spanish speaking a plus
