Position Overview:
As a dedicated RN Case Manager within the hospice sector, you will play a crucial role in delivering compassionate care to patients in their homes, while also providing support to their families. This position offers a unique opportunity to enhance your nursing skills in a flexible and autonomous environment.

Key Responsibilities:
- Plan, direct, and provide safe patient care tailored to individual needs.
- Engage in one-on-one interactions with patients, ensuring their comfort and well-being.
- Collaborate with families to support their emotional and physical needs during challenging times.

Schedule:
- Monday to Friday, 8 AM to 4:30 PM, with on-call weekend and holiday rotations.

Qualifications:
