Social Worker/Case Manager Role at D'Youville Life and Wellness Community

This is a challenging opportunity to join the interdisciplinary team at D'Youville Life and Wellness Community as a Social Worker/Case Manager. As a key member of our care team, you will work closely with patients/residents and their families to develop and implement personalized care plans. Your commitment to delivering high-quality, patient-centered care will make a significant difference in the lives of our residents.

The ideal candidate will have a minimum of two years of experience in social work/case management in a skilled nursing facility (SNF) and hold a BSW degree with a current MA social work license. You will be responsible for monitoring all care provided to patients/residents, confirming safe and timely discharges, and ensuring they receive the necessary services and equipment upon returning home. Your strong communication skills and ability to work collaboratively with healthcare professionals will enable you to effectively coordinate post-discharge care and address resource utilization.

This role offers a competitive compensation package, including $70K to $92K annually based on skills and experience.
