Job Overview

We are seeking an experienced Care Coordinator to join our team at South Middlesex Opportu.

About the Role:

  • This is a full-time position with an estimated annual salary of $60,000 - $70,000, depending on experience.
  • The successful candidate will work Monday to Friday, 9:00 am to 5:00 pm, with occasional travel to various housing properties in Western Mass.
  • You will be responsible for managing all aspects of the LTLC social services program, serving homeless and disadvantaged individuals.

Key Responsibilities:

  • Plan and develop program services according to client needs
  • Participate in weekly program staff and team meetings
  • Work closely with SMOC Housing Staff to ensure coordinated housing and services for each participant
  • Maintain monthly and annual statistical activity reports as requested by funding sources
  • Maintain direct contact with client population through continued service delivery and acting as back-up for direct care staff when necessary

Requirements:

  • Bachelor's degree required; Master's degree preferred
  • 1-3 years serving homeless population
  • Must have a valid driver's license
  • Excellent verbal and written communication skills
