Job Summary

We are seeking a skilled Medical Care Coordinator to join our interdisciplinary team in Lowell, MA. As a key member of our care coordination services, you will work closely with healthcare providers, social workers, and other professionals to ensure seamless care transitions and optimal health outcomes for our clients.

Responsibilities
  • Analyze client needs and develop personalized care plans to address medical, behavioral health, and social determinants of health
  • Collaborate with healthcare providers to coordinate authorizations, referrals, and service delivery
  • Provide education, consultation, and care coordination to active enrollees and their families
  • Document care coordination activities, data, and progress in CCA's electronic platforms
  • Participate in care transitions, post-discharge follow-up, and 24/7 rotating on-call coverage as needed
Requirements
  • Licensed Registered Nurse (RN) with at least two years of experience working with people with severe and persistent mental illnesses or medical coordination
  • Strong communication and interpersonal skills to establish supportive relationships with clients and their families
  • Ability to work effectively in a team environment and adapt to changing priorities
  • Computer literacy and proficiency in using software applications
  • Valid driver's license and reliable transportation
