Job Title: Senior Health Promotion and Testing Counselor

Care Resource is seeking a highly motivated and experienced Senior Health Promotion and Testing Counselor to join our team. As a key member of our prevention outreach and navigation program, you will play a critical role in assisting our supervisor with the coordination of prevention outreach and navigation programs.

Key Responsibilities:
  • Assist in the administration of programs, including monitoring staff working outside the office and maintaining accurate data collection and evaluations.
  • Conduct outreach activities in target communities to promote agency services and provide individual, group, or community-based health education and promotion.
  • Implement health education and promotion programs, including curriculum design, implementation, and evaluation.
  • Provide pre and post-test counseling for HIV, HCV, STIs, and health screenings, as well as perform HIV, HCV, and STI testing and health screenings.
  • Link clients testing positive to HIV primary care and make referrals to other services and service providers as needed.
  • Document client information and productivity using software, maintain accurate and timely chart documentation, and comply with all data and forms submission requirements.
Requirements:
  • Bachelor's degree in a related field, such as public health, health education, or a related field.
  • Minimum 2 years of experience in health promotion, education, and testing, preferably in a community-based setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality services.
