Job Summary

Care Resource is seeking a highly motivated and experienced Senior Health Promotion Specialist to join our team. As a key member of our prevention outreach and navigation program, you will play a critical role in assisting our supervisor with the coordination of prevention outreach and navigation programs, providing testing, counseling, and treatment access to clients, and promoting health education and prevention services.

Key Responsibilities
  • Assist in the administration of prevention outreach and navigation programs, including monitoring staff working outside the office and maintaining accurate data collection and evaluations.
  • Conduct outreach activities in target communities to promote agency services and provide health education and prevention services.
  • Provide pre and post-test counseling for HIV, HCV, STIs, and health screenings, and perform HIV, HCV, and STI testing on clients.
  • Ensure productivity standards are met in relation to the number of tests and workshop sessions, and work with outreach workers and other agency staff to link clients testing positive to HIV primary care.
  • Make referrals to appropriate services and service providers as needed, and maintain timely, accurate, legible, and clear chart documentation according to agency requirements.
Requirements
  • Bachelor's degree in a related field, such as public health, health education, or a related field.
  • At least 2 years of experience in health promotion, prevention outreach, and navigation programs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Valid driver's license and reliable transportation.
