The responsibilities include conducting home assessments, interviewing clients who are elderly or have disabilities, documenting findings, evaluating service needs, and formulating service plans. Additionally, you will verify information, clarify program benefits and eligibility criteria, refer clients to suitable services, and ensure that authorizations are accurately recorded in our automated systems.

You will also oversee and evaluate CCSE programs to ensure they effectively meet the needs of individuals.

Key Responsibilities:

  • Consistently attends work according to agency policies and performs additional duties as required.
  • Conducts interviews with applicants, service recipients, and relevant stakeholders, and performs home visits to evaluate service needs and provide information on available resources.
  • Performs reviews to assess eligibility and functional requirements.
  • Collects, verifies, and computes income and resources to determine financial eligibility for relevant programs.
  • Maintains comprehensive case records and inputs data into an automated system, creating a record for each service recipient.
  • Monitors service recipients through follow-up contacts to ensure their needs are being met.
  • Develops, coordinates, and reviews service plans in collaboration with clients, their families, provider agencies, and other state agency personnel, ensuring services are authorized accurately.
  • Prepares basic ongoing or special narrative and statistical reports.
  • Collaborates effectively with clients, their families, provider agencies, and other state agency staff to compile information and ensure accurate service authorization.

  • Strong oral and written communication skills.
  • Ability to prioritize tasks, set timelines, and meet deadlines.
  • Capability to build and maintain effective relationships with clients, colleagues, contract agency staff, and personnel from other federal or state agencies.
  • Proficiency in operating computers and general office equipment.
  • Knowledge of interviewing techniques to gather personal information and resolve discrepancies.
  • Familiarity with resources available for individuals who are elderly or have disabilities.
