Overview:


AltaPointe Health's Adult Residential Services are tailored for individuals experiencing severe and ongoing mental health challenges who need social rehabilitation and community assistance.

By integrating medical, nursing, and therapeutic services, our dedicated team offers care and support to over 200 adults requiring intensive service delivery.

The Adult Residential Services Program includes group living environments, which are monitored around the clock when residents are present.

The primary goals for each adult group home are to avert re-hospitalization and to optimize and sustain successful community integration. Our residential care emphasizes rehabilitation and reintegration into the community for individuals with serious mental health conditions.

Responsibilities:


Community Support Specialists assist supervisors in various tasks, including organizing and executing program activities, completing documentation, reviewing medication administration records (MARs), and facilitating the discharge and care coordination for residents.

They provide coverage for all staff, including their supervisor, and manage financial oversight, residents' appointments, transportation, and medication administration.

Essential Functions

1. Aids the supervisor in planning and executing program activities for the residential facilities.

2. Manages appropriate financial oversight with residents.

3. Coordinates residents' appointments and ensures suitable transportation arrangements are in place.

4. Assists the supervisor in timely completion of necessary reports (e.g., vehicle and facility inspections).

5. Provides coverage and direct services as required, including stepping in as program supervisor when necessary.


6. Aids in reviewing MARs to ensure effective medication management, including timely refills and necessary arrangements with healthcare providers for new prescriptions.

7. Assists in applying for residents' benefits.

8. Demonstrates the ability to identify crisis situations and proactively intervenes to de-escalate or resolve issues.

9. Collaborates effectively with treatment team members and external resources in delivering clinical services.

10. Advocates for residents to secure community resources.

11. Supports discharge planning and facilitates coordination of care to a less restrictive environment.

12. Documents services provided in a timely manner according to AltaPointe policies and program procedures.

13. Completes all necessary documentation for continuity of care.

14. Reports any incidents or driving violations to the supervisor and human resources.

15. Maintains clear and concise documentation of services provided, individual resident responses, and progress towards treatment objectives.

16. Prepares individualized and comprehensive biweekly summaries reflecting progress towards treatment goals.

17. Seeks supervision and consultation as needed.

18. Accepts and implements feedback for improvement.

19. Actively works to enhance leadership capabilities and provides positive and effective leadership.

20. Treats residents with respect, dignity, and compassion.

21. Upholds residents' privacy and confidentiality.

22. Maintains a pleasant and cooperative demeanor with others.

23. Assists residents and visitors as required.

24. Personal values do not hinder the ability to relate to and care for others.

25. Is sensitive to residents' needs, expectations, and individual differences.

26. Exhibits gentleness and calmness with residents and their families.

27. Actively participates in Performance Improvement initiatives.

28. Engages in AltaPointe committees as necessary.

29. Completes assigned tasks promptly.

30. Adheres to AltaPointe policies and procedures.

31. Attends regular training sessions and workshops as assigned.

32. Performs any other duties as assigned by the supervisor.

Qualifications:


1. A bachelor's degree in a mental health service-related field with one (1) year of experience in a direct mental health service-related area.


2. A bachelor's degree in a non-mental health service-related field with two (2) years of experience in a direct mental health service-related area.

3. A high school diploma/GED with three (3) years of experience working directly in a mental health residential setting.

4. Must possess a valid Driver's License and provide proof of current liability insurance.

5. Effectively demonstrates the application of adult-specific competencies as outlined below, to be attained within six (6) months of employment:

Knowledge of psychiatric disorders in the adult population

Knowledge of available community resources

Knowledge of substance abuse treatment issues, developments, and trends

Knowledge of Senior Adult issues for referral purposes

Knowledge of cultural diversity

Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic individuals, as well as non-violent crisis intervention practices

Knowledge of educational methods appropriate for adults with psychiatric disorders

Successful completion of MAC certification

Knowledge of all applicable community mental health standards (DMH, JC, Medicaid)

Knowledge of utilizing the Electronic Health Record

Successful completion of Defensive Driving.
