Compensation: $21.00-$28.00 (Dependent on Experience).
Work Schedule: Full-time, Monday to Friday, 8 am to 5 pm.


Key Responsibilities:

  • Manage all incoming communications and process referrals.
  • Assess incoming inquiries to identify required services and direct to appropriate programs or medical professionals as necessary.
  • Facilitate the registration of new patients utilizing electronic medical records (EMR).
  • Input new referrals into patient records within the EMR system.
  • Request essential information from referral sources.
  • Coordinate and process palliative care referrals.
  • Collaborate with oncology, primary care, palliative care, and transitions teams.
  • Verify Medicare and Medicaid eligibility for hospice services.
  • Confirm the licensure and NPI numbers of new referring physicians, entering this data into the EMR.
  • Follow up on all orders and documentation requests as required.
  • Provide clinical intake support and backup as needed, including entering medication information and conducting pre-admission calls.

Required Qualifications:

  • Familiarity with medical terminology is preferred.
  • Ability to interact tactfully with clients and the community.
  • Exemplary public relations skills.
  • Demonstrated ability to work independently, stay organized, and adapt to changing circumstances.
  • 2-3 years of experience in healthcare or a related field is preferred.
  • Proficient in Microsoft Office Suite, including Outlook, Word, and Excel.
  • Must pass background checks, drug screening, and TB testing.
  • For field roles, valid automobile insurance, a current driver's license, and reliable transportation are required.
