Job Summary

Key Responsibilities
  • Conduct face-to-face assessments and develop personalized care plans to meet the unique needs of each individual.
  • Collaborate with medical directors and interdisciplinary care teams to ensure seamless care coordination and effective management of physical and behavioral health needs.
  • Engage with individuals, their families, and support networks to identify and address gaps in care and develop strategies for optimal health outcomes.
  • Manage non-clinical needs of individuals with chronic illnesses, co-morbidities, and disabilities, ensuring cost-effective and efficient utilization of long-term services and supports.
  • Document short- and long-term service and support goals in collaboration with the individual's chosen care team.
  • Identify opportunities for alternative levels of service or other waiver programs to ensure optimal care and support.
  • Participate in process improvement initiatives and serve as a mentor, subject matter expert, or preceptor for new staff.
Requirements
  • Bachelor's degree in a healthcare-related field or equivalent experience.
  • Minimum of 2 years of experience working with a social work agency or equivalent experience.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to travel to worksites and other locations as necessary.
