Job Summary

Smith Arnold Partners is seeking a highly skilled and experienced RN to fill the role of Director of Case Management. As a key member of our team, you will be responsible for overseeing the administration, direction, and evaluation of Case Management services.

Key Responsibilities:
  • Direct and manage case management and social work activities to ensure high-quality care and compliance with regulatory standards.
  • Address and resolve patient, family, and staff satisfaction issues in a timely and effective manner.
  • Identify and intervene in specific length of stay (LOS) issues to optimize patient outcomes.
  • Develop and evaluate case management strategies for high-risk and high-volume patients.
  • Implement and evaluate concurrent utilization review practices to ensure efficient use of resources.
  • Effectively manage concurrent denials and appeals to minimize financial losses.
  • Analyze data to inform departmental activities and improve performance metrics.
  • Collaborate with other department leaders to ensure seamless integration of Case Management and Social Work services across the continuum.
  • Monitor and maintain readiness for Joint Commission on Accreditation of Healthcare Organizations (JCAHO), federal, and state regulatory requirements.
Requirements:
  • Bachelor's Degree in Nursing
  • Master's Degree in Nursing or related field (preferred)
  • Minimum 5-10 years acute care clinical experience
  • Minimum 5 years case management leadership experience
  • New York State Licensed Registered Professional Nurse
  • Certification in Patient Review Instrument (PRI) and Screen
  • Certification in Case Management
  • Certification in Hospital Management (highly preferred)
