Job Title: Population Health Coordinator

Join our team at Codman Square Health Center as a Population Health Coordinator, where you will play a vital role in improving patient care and enhancing clinical outcomes.

Job Summary:

The Population Health Coordinator will support the Health Center's Population Health Team in achieving enhanced clinical quality outcomes through quality improvement and intradepartmental coordination. This role involves updating the electronic health record (EHR) with quality data, performing EHR chart review, creating and utilizing patient registries, tracking patients to ensure test and appointment completion, and outreaching to patients.

Key Responsibilities:
  • Assist in gathering and analyzing healthcare quality data, such as clinical outcomes, patient satisfaction, and operational performance metrics.
  • Work with teams to identify trends and areas for improvement based on data insights.
  • Participate in performance improvement initiatives focused on enhancing healthcare processes, reducing waste, and improving care delivery.
  • Support initiatives aimed at improving the health outcomes of specific populations, with a focus on prevention, wellness, and addressing social determinants of health.
  • Collaborate with clinical, administrative, and operational staff to ensure that performance improvement strategies are aligned with patient care goals.
Requirements:
  • Currently pursuing a degree in public health, healthcare administration, nursing, or a related field.
  • Strong analytical skills and ability to interpret healthcare data.
  • Excellent written and verbal communication skills.
  • Detail-oriented and organized.
  • Ability to work independently and collaboratively within a team environment.
