Empower Lives as a Case Manager

At Unity House of Troy, we are seeking a dedicated Case Manager to join our Empire State Supportive Housing Initiative ESSHI Program. As a Case Manager, you will play a vital role in providing supported services to individuals living with mental illness, promoting independent living education, and advocating for consumers. Your expertise will be essential in coordinating care with other case managers and providers to wrap around services, while fostering a culture of inclusion and belonging.

Responsibilities:
• Provide supported services to individuals living with mental illness
• Support the Empire State Supportive Housing Initiative
• Provide independent living education
• Advocate for consumers
• Assist with housing placement and referrals
• Crisis intervention to program consumers

Requirements:
• High school diploma or GED
• At least 2 years' experience working with adults living with a mental illness and/or other high-risk populations
• Experience working with the homeless population a plus
• College degree in a related field preferred
• Valid New York State Driver License
• Reliable transportation
• Ability to work independently and as a team
• Bi-Lingual a great asset
