Home Based Crisis Intervention Specialist

Northern Rivers Family of Services is seeking a skilled Home Based Crisis Intervention Specialist to join our team. As a Home Based Crisis Intervention Specialist, you will provide in-home services to families and youth in crisis, assisting them in implementing behavior management techniques and crisis de-escalation skills. You will work closely with families to help them help their children, promoting safety and well-being in the home environment.

Responsibilities:

  • Deliver crisis intervention services in a home-based environment, responding to the needs of families in crisis.
  • Promote the safety of children, families, and communities through the delivery of crisis intervention services.
  • Tailor services to meet the unique needs of each family, scheduling sessions flexibly to accommodate their requirements.
  • Conduct comprehensive assessments of family strengths, values, skills, problems, needs, and barriers to goal attainment, collaborating with families to set goals and develop service plans.
  • Use a variety of teaching methods to help family members acquire, maintain, and generalize skills, advocating for and providing concrete services and supports as needed.
  • Collaborate with formal and informal community resources and systems that impact families, teaching family members to advocate for themselves and prepare them for transition and service closure.
  • Provide consultation to intra-agency service providers and community service providers, maintaining liaisons and relationships with external and internal providers, particularly as it relates to County Mental Health Departments working with the Home Based Crisis Intervention team.
  • Participate in the Continuous Quality Improvement process, collecting data and formulating recommendations and conclusions as assigned.

Requirements and Education:

  • Bachelor's degree in human services or Master's degree in Social Work or Mental Health Counseling, with at least two years of experience in a home-based and/or crisis intervention services program.
  • LMSW or LMHC licensure preferred.
  • Experience in the Homebuilder model of crisis intervention is preferred.
  • Must have a valid NYS driver's license.
