Position Overview:

The Cardiologist works in collaboration with a multidisciplinary care team to enhance the quality of care for our patients. This team includes physicians, advanced practice providers, medical assistants, behavioral health specialists, quality coders, and referral coordinators. Our integrated approach aims to provide exceptional healthcare experiences for seniors, focusing on superior care and improved outcomes through a concierge model, coordinated services, and strong community ties.

Key Responsibilities:

  • Assess and manage patients in alignment with established care standards.
  • Facilitate the coordination of patient services, including appropriate specialty referrals.
  • Serve as a vital source of medical knowledge within the care team.
  • Collaborate with primary care providers and nurse practitioners to ensure cohesive practice.
  • Maintain thorough medical documentation daily, partnering with quality coders for comprehensive records.
  • Adhere to policies set forth by Clinical Leadership.
  • Support initiatives aimed at heart failure management and reducing readmission rates.
  • Develop strategies for stroke prevention and manage care for patients with CKD stage 5.
  • Analyze test results and coordinate care with other medical specialists.

Essential Qualifications:

  • Graduation from an accredited MD or DO program.
  • Board Certification in Cardiology.
  • Possession of an active and unrestricted medical license.
  • Strong interpersonal and communication abilities.
  • Commitment to a team-oriented care model.
  • Basic proficiency in computer skills.
  • Patient-facing role requiring TB screening.

Preferred Qualifications:

  • Familiarity with HEDIS/STARS and other healthcare quality measures.
  • Experience in diverse community settings and with electronic health records.
  • Background in Medicare Advantage Global Risk Models.

Additional Information:

  • No on-call or weekend responsibilities.
  • This position does not involve pediatric care.

Reports To: Network Manager.
