Job Summary

Sage Health is seeking a highly skilled Cardiology Physician Partner to join our team-based care model. As a key member of our care team, you will partner with our physicians, advanced practice professionals, and other healthcare experts to provide high-quality care to our patients.

Key Responsibilities
  • Evaluate and Treat Patients: Evaluate and treat patients in accordance with standards of care, ensuring the highest level of quality and patient satisfaction.
  • Coordinate Patient Services: Assist in the coordination of patient services, including specialty referrals, cardiology subspecialists, and vascular surgeons, to ensure seamless care transitions.
  • Lead the Care Team: Act as an active participant and key source of medical expertise with the care team, providing guidance and support to ensure optimal patient outcomes.
  • Collaborate with Primary Care Physicians: Partner with primary care physicians and nurse practitioners to provide comprehensive care, displaying a professional and approachable demeanor.
  • Document Patient Records: Complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation.
  • Follow Policy and Protocol: Follow policy and protocol defined by Clinical Leadership, ensuring compliance with regulatory requirements.
  • Meet with Clinical Leadership: Meet with primary care physicians, nurse practitioners, and market medical director to discuss quality of care, review outcome data, and address medical records issues.
  • Focus on Direct Patient Care: Clinically focused on direct patient care, inclusive of patient-facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care.
  • Support Longitudinal Care Model: Support our longitudinal care model for heart failure, focusing on disease management and prevention.
  • Reduce Readmissions: Support readmission reduction initiatives for heart failure and CAD, working to improve patient outcomes.
  • Prevent Upstream Stroke: Create a practical framework for upstream stroke prevention, ensuring the highest level of patient care.
  • Coordinate CKD Care: Help coordinate care for CKD stage 5 patients needing AV graft placement prior to initiating hemodialysis.
  • Interpret Test Results: Interpret and document in detail test results to determine how effectively the heart and or cardiovascular system is functioning, including Echocardiograms and stress tests.
  • Coordinate with PCP: Coordinate patient care with primary care physicians.
  • Coordinate with Cardiology Subspecialists: Coordinate patient care with cardiology subspecialists, including interventional, EP, Thoracic, etc.
Requirements
  • Medical Degree: Graduate of an accredited MD or DO program of an accredited university.
  • Medical License: Current and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
  • Board Certification: Board Certification in Cardiology.
  • DEA License: Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Numbers: Medicare Provider Number and Medicaid Provider Number.
  • Interpersonal Skills: Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
  • Team-Based Care: Fully engaged in the concept of team-based care model.
  • Value-Based Care: Willingness and ability to learn/adapt to practice in a value-based care setting.
  • Patient Service: Superior patient/customer service.
  • Computer Skills: Basic computer skills, including email, EMR, and other relevant software.
Preferred Qualifications
  • HEDIS/STARS Knowledge: Knowledge of HEDIS/STARS.
  • CAHPS and HOS Knowledge: Knowledge of CAHPS and HOS.
  • Medically Underserved Communities: Experience working in medically underserved/culturally diverse communities.
  • Electronic Health Record: Experience working with an electronic health record.
  • Medicare Advantage Global Risk Models: Experience working in Medicare Advantage Global Risk Models.
