Position:

Patient Experience Coordinator
The Patient Experience Coordinator plays a vital role within the healthcare team, managing the administrative responsibilities that contribute to exceptional kidney care delivery.

This role involves proactively contacting patients via telephone to conduct satisfaction assessments.

Additionally, the Patient Experience Coordinator will handle all mailed surveys.

Key responsibilities include data entry and communication tasks, ensuring accurate data management and timely dispatch of communications and materials to patients.

Roles and Responsibilities
1. Conduct outbound calls to patients using provided scripts to complete satisfaction surveys.

2. Print, package, and send patient letters, postcards, educational materials, and other necessary documentation.

3. Handle incoming calls with a positive demeanor, fostering trust and rapport with patients.
4. Perform data entry for surveys, consent forms, and other essential documents.

5. Assist Monogram Health with various tasks to ensure the delivery of personalized kidney care and fulfill other assigned duties.

6. Uphold the mission and values of Monogram Health in all aspects of your work.
Position Requirements
- Demonstrated ability to learn and retain information in a fast-paced environment.
- Proven capacity to manage multiple priorities while meeting deadlines.
- Strong skills in overcoming patient objections.
- Dependable with a solid work ethic, attention to detail, and accuracy.
- Commitment to patient privacy by adhering to HIPAA regulations.
- Professional demeanor, polished presentation, and self-sufficiency.
- Open to feedback and coaching for personal development.
- Ability to thrive in a dynamic startup environment with a positive attitude.
- High School Diploma or GED required.
- Must be at least 18 years old.
- Proficiency in English is required; fluency in an additional language is advantageous.
- Excellent verbal, listening, and written communication skills are essential.
- Intermediate computer skills, including Microsoft Office and Salesforce Applications.
- Familiarity with the medical field, particularly care management or home health, is beneficial.
- A positive outlook and a desire to assist patients with chronic kidney disease in leading healthier lives.
- Ability to pass a background check and drug screening.
