Position Overview:

This role is pivotal in enhancing key performance outcomes and achieving system improvement objectives, including financial metrics, patient satisfaction, and clinical effectiveness as part of the care coordination framework. The position focuses on improving patient outcomes by minimizing hospital readmissions and managing care episodes for patients within specific populations or disease categories. Responsibilities include identifying suitable patients, fostering patient and family involvement in self-management, ensuring smooth transitions of care through effective communication, conducting educational sessions using the 'teach-back' method, and assisting patients in navigating the healthcare landscape.

Key Responsibilities:

  • Facilitates the assessment process for designated patient groups, documenting relevant information in collaboration with team members.
  • Acts as a liaison to enhance patient access between referring providers and Vanderbilt's healthcare professionals.
  • Engages with the clinical team to create a cohesive care plan that spans from initial hospitalization to the entire care episode.
  • Implements and oversees processes for pre-admission and post-discharge transitions, establishing partnerships with providers and agencies to optimize care.
  • Contributes to the creation and distribution of patient education materials, including tailored medication schedules with input from the pharmacy team.
  • Collaborates with multidisciplinary teams to facilitate discharge planning, sharing essential information about the patient's hospital journey and ongoing care needs.
  • Continuously evaluates care coordination practices and supports the development of evidence-based protocols.
  • Additional responsibilities may be assigned as needed.
