SUMMARY
Under the supervision of the Director of Intake & Admissions, the Patient Intake Coordinator manages inquiry and intake calls, assists with administrative tasks, and is cross-trained for various provider relations responsibilities. This role supports patient admissions, facilitates communication with providers (both internal and external), and ensures accurate data entry and documentation.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Acts as a representative of the agency to promote services and collaborate with provider relations representatives.
- Provides backup support to the Director of Intake & Admissions as necessary.
- Serves as a liaison and offers feedback from referral sources.
- Works collaboratively with physicians, social workers, discharge planners, and clinical coordinators to gather comprehensive information for treatment plans.
- Handles multiple calls, including inquiries, intake, family, and healthcare provider communications.
- Delivers accurate, consistent, and compassionate attention to each call.
- Maintains confidentiality for patients, families, and staff.
- Organizes, files, faxes, logs, and maintains data in an accurate and accessible manner.
- Inputs, manipulates, and retrieves data in the appropriate reporting format.
- Ensures timely routing of paperwork.
- Assists with administrative tasks as assigned by the Director of Intake & Admissions and the Director of Clinical Partnerships.
- Other responsibilities may be assigned as needed.
No supervision of agency staff or volunteers is required.
LANGUAGE SKILLS
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group settings to customers, clients, and other employees of the organization. Ability to respond to common inquiries from customers, clients, or other employees of the organization. Proficiency in computer and telephone skills is essential.
MATHEMATICAL SKILLS
Ability to perform basic arithmetic operations using whole numbers, common fractions, and decimals. Ability to compute rates, ratios, and percentages, and to interpret bar graphs.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions provided in written, oral, or diagram form. Ability to address problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
If required to drive to fulfill the duties of this position: a current driver's license and automobile insurance as mandated by state law.

CONTINUING EDUCATION
The agency requires this position to complete 6 hours of continuing education annually covering topics that will enhance the ability to fulfill the above responsibilities. Regulatory agencies may mandate additional hours for certain disciplines to maintain licensure or certification.

EDUCATION and/or EXPERIENCE
Associates Degree (A.A.) from a college or technical school; one year's related experience and/or training; or an equivalent combination of education and experience.
