Primary Care Physician Assistant

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at Providence ElderPlace in Oregon. As a member of our interdisciplinary team, you will provide comprehensive care and social services to older adults, working in an established program that coordinates all necessary care services to enable seniors to remain active within their communities.
Key Responsibilities- Manage a small panel of patients at your primary practice, providing high-quality, patient-centered care.
- Collaborate with physicians, nurse care managers, social workers, pharmacists, behaviorists, and other team members to ensure seamless care coordination.
- Participate in the development and implementation of individualized care plans, taking into account the unique needs and goals of each patient.
- Conduct home visits and visits in community-based settings, providing care and support to patients in their preferred environments.
- Work closely with the interdisciplinary team to ensure that patients receive comprehensive care, including medical, physical, and occupational therapy, lab tests and procedures, and home care services.
- Minimum of 1-year experience working with a geriatric population, preferably in a PACE (Program of All-inclusive Care for the Elderly) setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on providing high-quality, patient-centered care.
- Training and/or experience in palliative care is highly desirable.
