Key Responsibilities- Provide comprehensive primary care to adult patients, including routine check-ups, health screenings, and disease management.
- Collaborate with an embedded APP, on-site behavioral health provider, and clinical pharmacist to provide integrated care.
- Participate in case management and care coordination to ensure seamless patient care.
- Develop and maintain strong relationships with patients, families, and community partners.
