Position Overview
The Physician Assistant (PA) in Primary Care will provide essential medical care and treatment services, focusing on chronic disease management and health promotion for patients who may not thrive in a traditional clinic environment. Care will be delivered in patients' residences and through telehealth sessions.

Key Responsibilities
1. **Clinical Services**
- Deliver diagnosis, treatment, and follow-up care in collaboration with licensed physicians.
- Conduct comprehensive health assessments, identifying patient needs across medical, mental, and social dimensions.
- Develop and implement patient-centered care plans in coordination with multidisciplinary teams.
- Order diagnostic tests and medications, ensuring appropriate follow-up and reassessment.
- Provide compassionate guidance and high-quality palliative care aligned with patient and family goals.

2. **Administrative Duties**
- Maintain effective communication with medical staff regarding consultations and referrals.
- Participate in quality improvement initiatives and adhere to organizational policies and procedures.

Qualifications
1. **Required**
- Master’s degree in a primary care program for physician assistants or a related field.
- Licensed as a physician assistant in Oregon.
- DEA license and NCCPA certification.
- Valid driver’s license and acceptable driving record.

2. **Preferred**
- At least two years of relevant clinical experience.

Working Conditions
This role involves both indoor and outdoor work environments, with potential exposure to various hazards. The position requires travel outside the workplace, and the use of personal vehicles may be necessary.
