Position Overview
The Physician Assistant (PA) role is integral to our Primary Care department, providing essential medical care and treatment services with a focus on chronic disease management and health promotion for patients who may not thrive in traditional clinic settings.

Key Responsibilities
1. **Clinical Services**
- Conduct comprehensive health assessments, including physical and psychosocial evaluations.
- Diagnose and treat a variety of health conditions in collaboration with licensed physicians.
- Develop and implement patient-centered care plans, addressing both immediate and long-term health needs.
- Provide ongoing care for assigned patients, focusing on disease prevention and health maintenance.
- Deliver palliative care, ensuring alignment with patients' goals and preferences.

2. **Administrative Duties**
- Maintain accurate medical records in accordance with agency protocols.
- Participate in quality improvement initiatives and staff meetings.
- Coordinate case management activities for assigned patients, ensuring continuity of care.

Qualifications
Required:
- Master’s degree in a relevant field.
- Licensed Physician Assistant in Oregon.
- DEA license and NCCPA certification.
- Valid driver’s license and CPR certification.

Preferred:
- Minimum of two years of clinical experience in a related setting.

Working Conditions
This position involves both in-person and telephonic patient interactions, requiring travel to various locations as needed. Candidates should be prepared to work flexible hours, including evenings and weekends.
