About the Role

Vera Whole Health is seeking a highly skilled and compassionate Primary Care Physician to join our team as a PRN provider. As a provider at Vera, you will be focused on building strong relationships with your patients to address their needs beyond their symptoms.

You will meet with 10 to 15 patients each day for 30 to 60 minute appointments, supported by an entire care team, including health coaches, to deliver whole person care. This care model is designed to help people achieve optimum social, psychological, and physical well-being.

Key Responsibilities
  • Provide high-quality, patient-centered care to a diverse patient population
  • Build strong relationships with patients to address their needs beyond their symptoms
  • Work collaboratively with a team of healthcare professionals to deliver whole person care
  • Meet with patients for 30 to 60 minute appointments to discuss their health and wellness
  • Develop and implement personalized care plans to meet the unique needs of each patient

Requirements
  • MD, DO, or NP Board Certification with primary care, family medicine, or internal medicine background
  • Current, valid and unrestricted medical or ARNP license and valid DEA number
  • Minimum 3-5 years of primary care experience
  • Minimum 2+ years with case management emphasizing health prevention and population health management
  • Team-based care experience
  • Experience in coding and documentation, including Medicare Advantage and HCC
