Your Role:

As an Outreach Team RN at Imagine Pediatrics, you will cultivate positive, long-lasting relationships with pediatric offices, specialty practices, hospital discharge planners, social workers, home health agency nurses, and other community team members to enhance the engagement of eligible patients and their families. The Outreach Nurse is responsible for boosting patient and family involvement through direct outreach via phone and community engagement. You will collaborate with other members of the interdisciplinary team, including physicians, advanced practice providers, social workers, and navigators, as needed to enroll patients and caregivers.
  • Devise innovative strategies to enroll patients and families into our services.
  • Collaborate with the Director of Network & Engagement to build relationships with pediatricians, pediatric specialists, local hospitals, home health agencies, group homes, and clinics to promote engagement.
  • Manage relationships: Establish a regular meeting schedule with community partners to discuss our services, engaged patients/families, and eligible patients.
  • Develop strong working relationships with practice managers, primary care providers, discharge planners, and key provider stakeholders to establish formal business connections.
  • Comfortable conducting telephonic outreach to eligible families to enroll them in our program.
  • Utilize reporting and dashboards to prioritize telephonic and field-based outreach to members to enhance engagement.
  • Meet or exceed established monthly engagement goals.
  • Outreach to caregivers and assist in the onboarding process of our integrated technology platform.

Qualifications

First and foremost, you are passionate and dedicated to reimagining pediatric healthcare and ensuring that every child with complex medical conditions receives the care and support they deserve. You thrive in a dynamic environment and are adaptable to changing priorities. In this role, you will need:

Required:
  • Compact Licensed RN or eligibility for other state licensure.
  • Bachelor's degree in nursing from an accredited institution preferred.
  • A minimum of 2 years of direct clinical experience in PICU, Pediatric Emergency Department, and/or Pediatric Complex Care Clinic.
  • Bilingual in English and Spanish.

Preferred:

  • Prior experience in virtual healthcare.
  • Proficiency in Microsoft Excel, PowerPoint, and Tableau.
