POSITION SUMMARY


The Patient Care Coordinator is tasked with managing all incoming communications from both internal and external sources, ensuring a professional and courteous response.

This role includes verifying eligibility, assessing transportation needs, and determining the appropriate mode of transport. The Coordinator will utilize scheduling software to handle recurring, short-notice, and future transportation arrangements, while also troubleshooting issues with healthcare facilities and insurance personnel.

KEY RESPONSIBILITIES

  • Manage incoming calls and communications promptly.
  • Perform data entry, documentation, and email correspondence.
  • Collaborate with team members, other Access2Care staff, healthcare professionals, and providers.
  • Gain proficiency in Access2Care's transportation software.
  • Assist in coordinating recurring and urgent trips for eligible members.
  • Comply with all organizational policies and procedures.
  • Carry out additional responsibilities as assigned.

REQUIREMENTS

Experience:

  • At least 1 year of experience in a Dispatch/Communications Center is preferred.
  • Experience in customer service and phone communication is preferred.
  • Prior experience in dispatching or routing is advantageous.

Education:

  • A High School Diploma or equivalent (GED) is required.
  • At least six (6) months of relevant work experience within the last four years is required.

Skills:

  • Strong oral, written, and interpersonal communication abilities.
  • Exceptional organizational skills with a keen attention to detail.
  • Critical thinking and problem-solving capabilities.
  • Ability to manage multiple tasks and prioritize effectively.
  • Proficiency in Microsoft Office applications (Word, Excel, Outlook).
  • Capacity to multitask with minimal supervision.
  • Willingness to learn various software applications.
  • Outstanding customer service skills.
  • Team-oriented with a collaborative approach.
  • Strong interpersonal skills to maintain effective relationships across all levels of management.
  • Familiarity with medical terminology and services is preferred.
  • Experience in the healthcare sector is advantageous.
  • Typing speed of at least 30 words per minute.
  • Multilingual skills are a plus.
