SHIFT:

Responsibilities:


We seek dedicated employees who will be Champions of the Select Medical Way, which includes prioritizing the patient, enhancing the quality of life in the community, continuously developing and exploring new ideas, delivering high-quality care, and doing what is right.


The Patient Care Coordinator is responsible for conducting utilization reviews and managing resources, discharge planning, treatment plan oversight, and financial management, while also ensuring accurate medical record documentation.

You will report directly to the Director of Case Management and provide social work services as needed, in accordance with state regulations.

Develops and implements a patient-specific, safe, and timely discharge plan.

Conducts verification of utilization criteria reviews.


Builds relationships and collaborates with payor sources to ensure appropriate reimbursement for hospital services, promoting cost-effective care through focused resource management within the care plan.

Ensures compliance with facility-wide Utilization Management policies and procedures.

Coordinates UR compliance with Quality Management to ensure all licensure and accreditation requirements are met.

Maintains fiscal responsibilities, ensuring the department identifies and negotiates the maximum possible reimbursement to optimize insurance benefit coverage for patients. Reviews insurance verification forms to mitigate risk.

Facilitates multi-disciplinary team meetings involving physicians, nurses, respiratory therapists, and rehabilitation therapists.

Minimum requirements:
Bilingual

Current licensure in a clinical discipline as a Nurse (RN/LPN/LVN) or a Respiratory Therapist OR a current licensed/certified Social Worker per state guidelines

Prior experience as an RN/LPN/RT/SW/CM in an inpatient hospital setting dealing with critical care/acute care patients

(e.g.,

ICU, step-down, med-surg, vents)

Adequate experience in an acute medical case management setting and the confidence to manage and direct a care plan for chronically critically ill populations.

Preferred qualifications that will enhance your success:
Specific experience in Care Management and Discharge Planning is preferred.

Working knowledge of the insurance industry and government reimbursement.


Availability to work a minimum of 2 shifts per month on an 'as needed' basis - shifts would typically be scheduled on weekdays, from approximately 8 a.m. to 5 p.m.
