About the Role

Hospice of Florida is seeking a highly motivated and passionate Community Liaison to join our dynamic team serving the Clay County/St. Augustine area. As a key member of our team, you will play a vital role in promoting and positioning Hospice's patient care services to the medical community and partners.

Key Responsibilities
  • Determine primary decision makers and establish strong working relationships with members of the medical community and partners.
  • Promote and position Hospice's patient care services through regular visits, highlighting the quality of care and unique capabilities.
  • Build the brand of the hospice as the preferred provider in our service area, achieving and exceeding agency goals for growth and census management.
  • Secure new referrals, build census, and meet established goals through regular calls to physicians, facilities, case managers, discharge planners, and social workers.
  • Conduct initial meetings with patients and families, providing an overview of hospice services and philosophy, and obtaining consents for care.
Requirements
  • Bachelor's degree in a related field (additional experience may be substituted for educational requirement).
  • Minimum two years of sales/marketing experience in healthcare, with hospice/palliative marketing experience strongly preferred.
  • Prior clinical experience is preferred.
  • Ability to establish and maintain working relationships with key staff, marketing contracts, and clinical staff.
  • Proven ability to generate and maintain relationships with new and existing accounts, including physicians, healthcare professionals, and community agencies.
