Community Options is a national non-profit agency dedicated to providing person-centered services to individuals with disabilities in 12 states. We are seeking a qualified Disability Support Professional to join our team in Brooklyn, NY, who can provide tailored care to meet the diverse needs of individuals with intellectual and developmental disabilities.

Job Summary

The Disability Support Professional will assist the Executive Director in developing and maintaining individual service plans that meet the needs of people with disabilities and state and area program guidelines. This role requires a strong focus on community-based support services, with a commitment to delivering high-quality care that promotes the autonomy and dignity of our clients.

Responsibilities
  • Service Plan Development: Develop individualized service plans that cater to the unique needs of participants, ensuring that their goals and objectives are met.
  • Admission and Discharge Coordination: Coordinate the admission and discharge of participants, ensuring a smooth transition and minimizing disruption to their care.
  • Liaison and Support: Act as a liaison between area programs and clinical consultants, providing support and supervision to staff regarding compliance and clinical guidelines.
  • Record-Keeping and Reporting: Maintain accurate and up-to-date records, including incident and accident reports, goal progress notes, and billing justification forms.
  • Incident Response and Prevention: Facilitate the selection of clinical consultants and monitor their activity, ensuring that participants receive the necessary support and services to prevent incidents and ensure a safe environment.
Requirements
  • Education and Experience: A Bachelor's degree and two years' experience in implementing community-based support services for individuals with disabilities.
  • Qualifications: QIDP qualified, with a valid driver's license and good driving record.
  • Skills and Abilities: Excellent organizational skills, with the ability to set priorities and follow through under time constraints. Effective communication skills, with the ability to work collaboratively with staff, participants, and other agencies.
Working Conditions

The Disability Support Professional will work in residential programs, day programs, and in the community, with a flexible schedule that may include evening and weekend hours. The role may require on-call duties, frequent lifting and physical exertion, and exposure to various medical conditions and communicable diseases.
