Hospice RN Case Manager Opportunity

We are seeking a skilled and compassionate Hospice RN Case Manager to join our team at Trinity Health At Home. As a Hospice RN Case Manager, you will provide professional nursing care to our hospice clients, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Assess and manage patient symptoms, collaborating with the physician and other team members as needed
  • Develop and implement individualized care plans, ensuring patient-centered care and comfort
  • Provide education and support to patients and their families, addressing grief and loss issues
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
Requirements:
  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the State of Georgia
  • One (1) year experience as a professional acute care nurse
  • Home care experience preferred but not required
