Hospice RN Case Manager Job Summary

We are seeking a skilled and compassionate Hospice RN Case Manager to join our team at Trinity Health At Home. As a key member of our hospice care team, you will provide professional nursing care to our clients, working closely with physicians and other healthcare professionals to ensure the best possible outcomes.

Key Responsibilities:
  • Assess client needs and develop individualized care plans
  • Provide direct patient care, including wound care, medication management, and symptom control
  • Collaborate with interdisciplinary teams to coordinate care and services
  • Communicate effectively with clients, families, and healthcare providers
  • Manage patient records and maintain accurate documentation
Requirements:
  • Graduate of an accredited nursing program
  • One year of experience as a professional acute care nurse
  • Home care experience preferred, but not required
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
