Job Summary

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will partner with our Director of Patient Care Services to provide high-quality care to our patients and their families.

Responsibilities
  • Provide one-on-one support to patients and their families to address everyday needs
  • Conduct initial and ongoing assessments of patient and family needs to develop and implement effective care plans
  • Administer medications and treatments as prescribed by the physician, and provide health care instructions to patients as needed
  • Record important health information, including temperature, pulse, respiration, and blood pressure
  • Collaborate with our interdisciplinary team to ensure comprehensive care and support
Requirements
  • A Registered Nurse license in good standing
  • 1 year of experience in hospice or home health preferred
  • A valid driver's license
  • Current CPR certification
  • Excellent observation, verbal, and written communication skills
