Contingent Hospice RN Case Manager Position Summary

We are seeking a skilled and compassionate Hospice RN Case Manager to join our team at Mary's Home Care and Hospice in Athens, GA. As a member of our interdisciplinary team, you will provide professional nursing care to our hospice clients, working collaboratively with physicians and other healthcare professionals to ensure the best possible outcomes.

Key Responsibilities:
  • Assess client needs and develop individualized care plans
  • Provide direct patient care, including symptom management and education
  • Collaborate with the interdisciplinary team to coordinate care and services
  • Manage patient records and communicate with healthcare providers
  • Provide support and guidance to hospice aides and LPNs/LVNs as needed
Requirements:
  • Graduate of an approved nursing education program
  • One (1) year experience as a professional acute care nurse
  • Home care experience preferred but not required
