Job Summary

As a Registered Nurse Case Manager, you will partner with our Director of Patient Care Services and Interdisciplinary Teams to further that mission. These passionate teams will depend on you to care for and support our patients during their hospice journey.

About the Role
  • Quality Time with Patients: Spend quality one-on-one time with patients and their families to support everyday needs.
  • Patient Assessments: Care for patients by providing initial and ongoing assessments of patient and family needs.
  • Care Planning: Create and initiate care plans through assessing the history of current and previous illnesses.
  • Health Record Keeping: Record important health information including temperature, pulse, respiration, and blood pressure.
  • Medication Administration: Administer medications and treatments as prescribed by the physician.
  • Health Education: Provide health care instructions to the patient as appropriate.
  • Symptom Management: Record symptom management changes and outcomes.
  • Clinical Notes: Complete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given.
Qualifications
  • Enthusiasm for Patient Care: An enthusiasm for providing the best possible care to patients and their families.
  • Registered Nurse License: A Registered Nurse license in good standing.
  • Hospice Experience: 1 year of experience in hospice or home health preferred.
  • Valid Driver's License: A valid driver's license.
  • CPR Certification: Current CPR certification.
  • Excellent Communication Skills: Excellent observation, verbal, and written communication skills.
