We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Trinity Health At Home. Key Responsibilities:
  • Assess and evaluate the physical, emotional, and spiritual needs of our hospice clients
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Provide education and support to clients and their families to promote optimal health outcomes
  • Collaborate with our interdisciplinary team to ensure seamless care coordination
  • Manage and coordinate the delivery of hospice services, including medication management and symptom control
Requirements:
  • Graduate of an approved nursing education program
  • One (1) year experience as a professional acute care nurse
  • Home care experience preferred but not required
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
