Care Coordinator

Job Summary:
The Care Coordinator will be responsible for coordinating the provision of services to individuals with complex needs, ensuring that they receive comprehensive and holistic care. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations and multidisciplinary teams.
Key Responsibilities:
- Obtain required enrollment consents from individuals or legal guardians
- Complete initial and ongoing needs assessments to determine the individual's most appropriate level of care coordination
- Develop and implement individualized plans of care, ensuring that services are coordinated and aligned with the individual's needs and preferences
- Monitor and evaluate the effectiveness of care coordination, making adjustments as needed to ensure optimal outcomes
- Collaborate with healthcare providers, social services, and other stakeholders to ensure seamless care transitions and coordination
- Provide education and support to individuals and their families, promoting self-advocacy and empowerment
- Document all interactions and interventions in a timely and accurate manner, utilizing designated care coordination portals and electronic health records
Requirements:
- Bachelor's degree in a related field, such as social work, nursing, or healthcare administration
- Minimum two years of experience in care coordination, healthcare, or a related field
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and multidisciplinary teams
- Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines
- Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments
Work Environment:
This position is located in Brooklyn, with some travel to other boroughs. A valid NYS driver's license is required if using a personal vehicle or agency vehicle for agency business.
Salary:
Salary will be commensurate with experience and skills.
