Job Title: Home Care Intake Coordinator

At True Care NY, we are seeking a highly skilled and compassionate Home Care Intake Coordinator to join our team. As a key member of our intake team, you will play a vital role in facilitating the enrollment process for our clients, ensuring seamless continuity of care.

Job Summary:

The Home Care Intake Coordinator will work closely with our Community Outreach Representatives and potential clients to process referrals, obtain necessary documentation, and assist with Medicaid enrollment. This role requires strong communication and organizational skills, as well as the ability to work effectively in a team environment.

Responsibilities:
  • Process referrals and ensure timely enrollment of clients in home care services
  • Liaise with partnering contracts to discuss member eligibility and start care processes
  • Obtain necessary documentation and assist with Medicaid enrollment for new patients
  • Follow up with potential patients and contracts to ensure smooth intake and enrollment processes
  • Manage the referral intake process from initiation through admission statuses
  • Analyze weekly status update reports and complete necessary paperwork
  • Maintain an active caseload with an appropriate quota and follow processes to get potential patients started with services
  • Schedule assessments and reassessments of applicants and enrollees using HHA eXchange
  • Communicate effectively with the Program Manager, team members, and the Marketing team
  • Exchange information with various departments to schedule assessments and gather required clinical and demographic data from patients and sources
  • Create, obtain, and track all communication notes for prior and present authorizations
Requirements:
  • Bilingual in Spanish is required
  • High School diploma/GED equivalent
  • Experience in Salesforce and HHA eXchange
  • Personable and professional with excellent communication skills
  • Compassionate and patient with a strong interest in helping potential patients enroll in home care programs
  • Ability to work in a team-building environment
