Position Overview: The role of the Eligibility Specialist is crucial in managing and facilitating client services programs, ensuring proper documentation and follow-up.
Key Responsibilities:
1. Assess all clients for program eligibility while identifying their needs through comprehensive interviews.
2. Conduct in-person evaluations and periodic reassessments every six months.
3. Verify that all clients referred to services meet eligibility criteria prior to service delivery.
4. Investigate and confirm clients' insurance coverage.
5. Assist with referrals and coordinate services in line with agreements established with partner organizations and governmental programs.
6. Maintain the accuracy of eligibility records through a thorough audit review process.
7. Document all client interactions in software applications, complete and submit billing documentation, and manage physical client files.
8. Adhere to all program requirements and stay informed of any changes.
9. Collaborate with clinic team members to ensure seamless patient service coordination.
10. Process necessary documentation for state and local funding sources as required.
11. Uphold confidentiality regarding patient and family information in accordance with professional standards.
Additional Responsibilities:
1. Be cross-trained in prevention, outreach, and testing initiatives for effective team collaboration.
2. Cultivate and maintain networking relationships with community and support resources.
3. Implement safety, environmental, and infection control protocols.
4. Promote and embody the values and mission of CAN Community Health, adhering to all policies and procedures.
5. Engage in ongoing professional development to enhance knowledge and skills related to HIV/AIDS.
6. Participate in the continuous quality improvement of services provided to clients.
7. Utilize resources and supplies efficiently and responsibly.
8. Exhibit courtesy and respect in all interactions with clients, staff, and visitors.
9. Maintain a professional demeanor in all job-related tasks.
10. Perform any other duties as assigned.
Reporting Structure: Reports to the Ryan White Program Coordinator.
Qualifications:
1. High School Diploma or GED.
2. Basic understanding of HIV/AIDS prevention and treatment.
3. Completion of Basic Annual Safety Training, HIV 104, 500, and 501.
Physical Requirements:
1. Maintain a neat and professional appearance.
2. Sedentary role, requiring exertion of up to 10 lbs. occasionally and/or a negligible amount of force frequently or constantly to move objects, including the human body. Primarily involves sitting, with occasional walking or standing.
3. Ability to express and exchange ideas through spoken word, with visual and auditory acuity.
Knowledge, Skills, and Abilities:
1. Valid driver's license.
2. Strong communication and interpersonal skills with patients, visitors, and staff.
3. Visual and auditory acuity.
4. Proficient in problem-solving techniques, self-motivated, organized, and capable of managing multiple tasks simultaneously.
5. Ability to read, comprehend, follow, and enforce safety protocols.
6. Demonstrated organizational skills.
7. Willingness to utilize computer software programs.
8. Capable of coordinating several tasks concurrently.
9. Ability to work with minimal supervision.
10. Willingness to travel for conferences, training, and meetings.
