Job Description

Job Title: Eligibility Screening Specialist for Supportive Services for Veteran Families

Job Summary: We are seeking a highly motivated and detail-oriented Eligibility Screening Specialist to join our team at Changing Homelessness, Inc. The successful candidate will be responsible for receiving, coordinating, and distributing referrals for the Supportive Services for Veteran Families (SSVF) program. This role requires strong interpersonal and communication skills, as well as the ability to work independently and as part of a small team.

Key Responsibilities:

  • Receive and respond to referrals for the SSVF program
  • Screen referrals by phone or in person for eligibility and assign referrals in coordination with the Director of Veteran Services
  • Track all referrals and communicate outcomes to referral sources
  • Respond to inquiries from clients and the community
  • Complete referrals to other community agencies for ineligible clients
  • Maintain detailed spreadsheets related to client eligibility for reporting purposes
  • Partner with HUD/VASH program to ensure effective coordination of services
  • Perform other duties as assigned

Requirements:

  • Associates degree in a related field or equivalent work experience
  • Minimum of 2 years of social services experience preferred
  • Must be eligible to work within the U.S. and provide supporting documentation
  • Must pass a Level II background check
  • Must pass a federal-level drug screen
  • Must have a clean 3-year driving history
  • Must have a valid driver's license and a registered vehicle without known issues or faults

  • Strong interpersonal and communication skills
  • Skill in organizing resources and establishing priorities
  • Working knowledge of community resources
  • Ability to work independently and as part of a small team
  • Knowledge of federal, state, and/or community funding sources and mechanisms
  • Ability to communicate effectively, both orally and in writing
  • Knowledge of grant funding policies and procedures
  • Ability to maintain confidentiality of client information

Physical Demands:

  • Periods of walking, standing, or sitting in an office or field environment
  • Limited physical effort required
  • Ability to operate a motor vehicle
