Job Title: Family Services Planning Team Coordinator

Children's Home Society of Florida is seeking a highly motivated and experienced Family Services Planning Team Coordinator to join our team. As a key member of our Family Services Team, you will play a critical role in coordinating the daily operations of our program, ensuring the delivery of high-quality services to children and families in need.

Job Summary:

The Family Services Planning Team Coordinator will be responsible for coordinating the daily operations of the Family Services Team program, including reviewing applications for eligibility, performing data entry, coordinating staffings with community providers, and developing partnerships with outside mental health providers. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Coordinate the daily operations of the Family Services Team program, including reviewing applications for eligibility and performing data entry.
  • Coordinate monthly staffings with community providers to staff cases for funding through the FSPT program.
  • Develop partnerships with outside mental health providers, facilitate contract preparation, and monitor for compliance.
  • Maintain FSPT files in compliance with required standards.
  • Monitor FSPT clients' progress and determine need for re-authorization of services.
  • Prepare FSPT authorizations within the monthly allocated budget.
  • Provide education in the community related to FSPT and Targeted Case Management in an effort to increase participation.
  • Review monthly billing for compliance and quality review.
  • Facilitate meetings with community-based agencies.
Requirements:
  • Bachelor's degree in Human Services, Social Work, Psychology, or related field, required.
  • Florida Individual Children's Mental Health Targeted Case Management Certification, required.
  • Florida Driver's License within 30 days from hire required, with daily access to a reliable and insured vehicle.
  • Three years of experience in targeted case management, required.
  • One year of experience with utilization and/or quality management, preferred.
