We are seeking a skilled Nursing Leadership Floor Supervisor - Resident Care Manager to join our team at Avamere Rehab Beaverton.

Key Responsibilities:

  • Develop and implement written plans of care for each resident, ensuring they align with medical and nursing treatments.
  • Complete and revise the Minimum Data Set (MDS) with accuracy, including implementing Continuous Assessment Alerts (CAA's) and Triggers.
  • Maintain daily rounds to ensure nursing personnel perform their work assignments efficiently.
  • Review nurses' notes to guarantee they provide informative and descriptive documentation of nursing care and resident responses.
  • Establish and maintain effective working relationships with all facility personnel to meet resident needs.
  • Participate in discharge plan reviews and prepare reports for the Care Plan Committee as directed.
  • Delegate, train, evaluate, and support RN, LPN, and CNA staff.
  • Provide direct nursing care when necessary.
  • Assist the Director of Nursing Services and fill in as needed.
  • Engage in facility surveys by authorized government agencies.
  • Recommend new or modified procedures, policies, methods, education, reference materials, and general nursing practices to ensure high-quality patient care.
  • Attend continuing education programs to stay updated on professional developments and participate in/provide leadership for in-service training.

Requirements:

  • A nursing degree from an accredited college or university.
  • A current, unencumbered, active license to practice as an RN in this state.
  • A minimum of 1 year of experience as a supervisor in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Knowledgeable about nursing and medical practices, laws, regulations, and guidelines pertaining to nursing care facilities.
  • Experience with Electronic Medical Records and computer documentation systems.
  • The ability to maintain confidentiality of all resident care information in accordance with HIPAA guidelines.

Salary: Estimated annual salary: $80,000 - $110,000 depending on qualifications and location.
