Key Responsibilities:

  • Patient Care:
    • Conduct initial assessments of patients and families to determine home care needs.
    • Develop and implement individualized care plans to meet the unique needs of each patient.
    • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
    • Counsel patients and families on meeting nursing and related needs.
    • Provide health care instructions to patients as appropriate per assessment and plan of care.
  • Communication:
    • Prepare and maintain accurate and comprehensive clinical notes.
    • Communicate with physicians and other healthcare professionals to coordinate patient care.
    • Collaborate with community health-related persons to ensure seamless care coordination.
  • Additional Duties:
    • Participate in on-call duties as defined by the on-call policy.
    • Ensure that necessary equipment and services are available to patients.
    • Supervise and evaluate home health aide care provided.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current licensure in the state and CPR certification.
  • One to two years of recent acute care experience in an institutional setting.
  • Bachelor's degree, with one year of home health care experience preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, or transfer patients, and ability to lift supplies and equipment.
  • Visual acuity and hearing to perform required nursing skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization's requirements.
