The Family Support Specialist plays a crucial role in delivering client-focused services within the Preventive Services Department. This includes essential support activities such as interviewing clients, gathering information about family dynamics, and directly assisting children and family members in various capacities, including:
Responsibilities:
Collaborate closely with Case Planners to deliver comprehensive services to individuals and families. Attend joint home visits with Case Planners and Child Protective Services at the initiation of cases.
Assist and model effective strategies for parents (caretakers) and children to achieve their service plan objectives. Engage in supportive services such as recreational and educational programs that promote behavioral goals for children.
Request and document all school and medical reports in Connections using the appropriate progress note template, ensuring all important documents are filed in case records.
Support families in accessing both in-house and community-based services to address issues that may place a child at risk of foster care placement, while promoting maximum safety, supervision, and support for children and their families.
Enter progress notes in CONNX within five days of occurrence, utilizing the progress note template. Submit draft copies of progress notes weekly to the supervisor and ensure all printed progress notes are filed appropriately. The quality of progress notes must be informative, descriptive, and purpose-driven.
Conduct required home visits, including collateral caregiver homes where children are cared for during the month, and maintain collateral contacts to meet ACS minimum contact mandates and specific model requirements.
Prepare the monthly PROMIS report and generate necessary case work contact reports, supervisor reports, and characteristics reports.
Participate in mandatory weekly individual and unit supervision, attend all required trainings and general staff meetings, and serve on assigned committees.
Review files for necessary updated documentation and assessments to ensure compliance with ACS, OCFS, and Rising Ground standards.
Provide ongoing coverage to the receptionist area as assigned in a rotation schedule.
Other duties as assigned.
Qualifications:
Bachelor's degree preferred.
High School Diploma or GED equivalent.
Prior experience in a child welfare environment is preferred.
A minimum of two years of experience as an Office Assistant is required.
Strong organizational and computer skills are essential, including proficiency in Microsoft Word and Outlook; familiarity with Excel is a plus.
A valid driver's license is a plus.
Bi-lingual (English and Spanish) is preferred.
This position is primarily in-office, with one remote day every two weeks.
Standard work week of 35 hours.
