Family Support Service Specialist

Job Overview:
We are seeking a highly skilled and compassionate Family Support Service Specialist to provide exceptional support services to our clients. As a Family Support Service Specialist, you will be responsible for integrating family goals and services with the tasks of all team members, providing family psychoeducation individually and in groups, and assessing the nature of family and social relationships.
Key Responsibilities:
- Assess the nature of family and social relationships and design interventions to promote progress towards familial and social wellness.
- Lead discussions and associated interventions around family issues and incorporate findings into service plans.
- Engage clients' families, significant others, and/or others identified by the client to provide education regarding mental illness and system resources.
- Provide networking and linkages between programs, mental health providers, and require both telephone and face-to-face contact.
- Develop treatment plans with the input of other members of the ACT team and the consumers.
- Coordinate case management services to ensure comprehensive development of an individual's service plan.
- Coordinate services with other specialists on the ACT Team, such as Substance abuse specialist, Employment specialist, Nurse, and Case manager.
- Attend and participate in all team meetings.
- Responsible for input into CAIRS upon admission, bi-annually, and discharge.
- Act as a clinical liaison between the team and outside entities.
- Document any progress notes in EHR within 48 hours of all client and collateral contacts.
- Ensure all documentation is completed in a timely fashion and remain in compliance with agency, OMH, and AOT regulations.
- Communicate pertinent consumer and operational information to the Team Leader, Deputy Director, Division Director, and Regulatory bodies.
Salary and Benefits:
The salary for this position is estimated to be in the range of $60,000.00 - $70,000.00 per year. In addition to a competitive salary, we offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
Requirements:
- Master's degree in social work or related human service field.
- One year of training and/or experience in family assessment and intervention, psycho-education, and/or other family support services, including cognitive-behavioral strategies.
- Experience working with behavioral health population preferred.
- Computer proficiency, including the ability to use email, navigate, and enter all documentation into electronic medical record.
