As a leading provider of services for multiply disabled adults experiencing chronic homelessness, DESC supports nearly 2,000 individuals daily. Position Summary:

The Mobile Response Team (MRT) offers round-the-clock mental and behavioral health services at Health through Housing (HtH) facilities and designated Permanent Supportive Housing (PSH) locations throughout King County. The primary objective of this program is to deliver immediate crisis behavioral health services and short-term behavioral health case management through mobile intensive response teams, assisting individuals in navigating available behavioral health and community service options. The MRT comprises a mobile interdisciplinary team, including certified peer counselors, behavioral health case managers, substance use disorder professionals, mental health professionals, and Narcan-waivered medical staff.

The Mental Health Professional (MHP) Case Manager will respond to crisis situations originating from designated housing sites. Responsibilities include conducting risk assessments, de-escalating crises, formulating safety plans, and collaborating with clients and housing staff to ensure safety.

Key Responsibilities:

  • Conduct timely outreach services to individuals in crisis, in collaboration with housing staff.
  • As part of a multidisciplinary team, perform risk assessments, develop safety plans, coordinate care, and assess for substance use to enhance stabilization and engagement for adults in crisis.
  • Facilitate referrals to MRT case management for ongoing support, establishing and implementing plans to engage clients in relevant services and resources.
  • Provide follow-up services to coordinate care with case managers, housing staff, and other support systems, ensuring linkage to services for program participants.
  • Complete all required data collection and documentation for the Project and DESC.
  • Build and maintain collaborative relationships with service providers for the target population.
  • Adhere to program research and evaluation protocols.
  • Follow all agency policies and procedures, as well as relevant Washington Administrative Code and Revised Code of Washington titles.
  • Advocate for clients within their communities, ensuring their needs are met and rights upheld.
  • Participate in crisis intervention and de-escalation during emergencies, assisting staff as needed to maintain a safe environment.
  • Perform other duties as assigned.

Living Conditions:

  • Coordinate support for clients residing in DESC housing, focusing on maintaining healthy living conditions through outreach and skill-building.

Physical Demands:

The physical demands outlined here represent those that must be met by an employee to successfully perform the essential functions of this role. Reasonable accommodations may be made for individuals with disabilities. While performing job duties, the employee will be required to sit, drive, walk, stand, communicate with colleagues and vendors, lift and carry items weighing up to 10 pounds, and operate computer systems. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The role may involve considerable stress and requires working indoors and outdoors in various weather conditions.

DESC is committed to fostering diversity in the workplace and promotes equal employment opportunities for all staff members and applicants. The Agency will not discriminate against any employee or applicant based on race, creed, color, sex, gender, sexual orientation, age, national origin, caste, marital status, or the presence of any sensory, mental, or physical disability in any employment practice, unless based on a bona fide occupational qualification. Minorities and veterans are encouraged to apply.

Minimum Qualifications:

  • Eligible for a Licensed AAC credential or any other credential meeting RCW requirements to act as a Mental Health Professional.
  • Master's degree in social work, psychology, or a related behavioral science, or a Bachelor of Nursing degree with a mental health specialty.
  • Valid Washington State driver's license and insurable driving record.
