Nurse Care Specialist

Responsibilities:
* Provide individualized care to clients, following their personalized care plans and promoting their dignity and independence.
* Monitor clients' vital signs and report any abnormalities to the case manager and physician.
* Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
* Collaborate with other healthcare professionals to ensure comprehensive care and support.
* Maintain accurate records of client care and communicate effectively with family members, case managers, and physicians.
* Ensure infection control policies are upheld and report any potential communicable diseases or infections.
* Order supplies and medications as needed to ensure adequate stock levels.
* Monitor incidents involving clients and complete incident reports in a timely and accurate manner.
* Maintain medical equipment and report any faulty equipment to the Durable Medical Equipment team.
Requirements:
* Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
* Knowledge of individual service plans and understanding of client rights and health needs.
* Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
* CPR certification.
* Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
* In-depth understanding of patient care policies and regulations.
* Familiarity with symptoms and signs of improvement for several medical conditions.
