About the Role

We are seeking a skilled Nurse Care Specialist to join our team at ResCare Community Living. As a key member of our healthcare team, you will be responsible for providing high-quality care and support to our clients.

Key Responsibilities
  • Team Collaboration: Attend and actively participate in weekly team meetings to ensure seamless communication and coordination of care.
  • Client Care: Provide rehabilitation and coordinate medical needs for clients, ensuring their physical and emotional well-being.
  • Medication Management: Monitor and administer medications as ordered by the physician, ensuring accurate and timely administration.
  • Financial Management: Assist in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures to ensure compliance and efficiency.
  • Assessment and Planning: Conduct assessments and approve plans of care and service delivery methods to ensure optimal client outcomes.
  • Training and Development: Perform competency and skills evaluations on employees who provide direct care, ensuring they meet the highest standards of care.
  • Communication: Inform the physician and staff of changes in the client's medical or mental condition and needs, ensuring timely and effective communication.
  • Documentation: Write progress notes, summaries, and discharges to ensure accurate and comprehensive documentation of client care.
  • Treatment and Rehabilitation: Implement treatment and rehabilitative activities and strategies to promote client independence and well-being.
  • Incident Reporting: Report significant/major incidents or accidents and notify proper authorities of circumstances or situations that present possible health hazards.
  • Client Choice and Independence: Follow consumer health plans and promote client choice and facilitate independence in daily living activities.
Requirements
  • Age Requirement: Must be 21 years of age.
  • Education: Holds a bachelor's degree in nursing or related health and human services field from an accredited college or university, or an RN from an accredited two-year program.
  • Driver's License and Insurance: Valid driver's license and insurance required.
